🎥 Bringing Support Plans to Life: The Power of Video Support Plans


We are always looking for meaningful ways to ensure the people we support are truly at the heart of everything we do. One of the ways we are bringing this to life is through the introduction of Video Support Plans, a powerful addition to our existing written plans.


Support plans have always been an essential part of delivering person-centred care. They are active documents; continually reviewed, updated and shaped around the individual as their needs, preferences and goals evolve. But we asked ourselves: how can we go further? How can we make support plans even more personal, engaging and impactful?


The answer came through co-production, led by our Moorview Matters group. Together, we developed the idea of Video Support Plans - giving the people we support a platform to share, in their own words, how they want to be supported. It reflects our commitment to listening, learning and continuously improving the way we deliver support, giving people ownership of how their support is understood, building confidence and strengthening relationships.


🗣️ Giving People a Voice


At the heart of every Video Support Plan is one simple but powerful principle:the person is the expert in their own life.


These videos allow individuals to speak directly to their support teams, especially new team members, sharing what matters most to them, what helps them feel safe, and how they like to be supported.

For some, this might mean speaking on camera. For others, we adapt the approach, using prompts, visuals, or support from someone they trust to ensure their voice is still clearly heard. The process is flexible and inclusive, designed to suit different communication styles and preferences.


🎬 The Video Planning Process


Creating a Video Support Plan is a thoughtful and collaborative process.

It often begins with a storyboard, where the person can explore what they want to share and how they want to tell their story. This helps them take ownership of the content and feel confident about the process.


Filming typically takes place in an interview-style format, supported by the person's Key Worker or someone who knows the person well. This familiar presence helps create a relaxed environment where people feel comfortable expressing themselves.

Videos can also include:

  • Clips from family members or other team members offering additional insight

  • Photos or short clips that bring activities, routines or preferences to life

  • Visual prompts to support understanding and engagement

Every video is unique - because every person is.


❓ The Questions That Matter


To guide the conversation, we focus on key questions that help build a deeper understanding of the individual, such as "What makes me feel safe?" and "What are my hopes and dreams?". These questions are incredibly important. They move us beyond task-based support and into Active Support and Person-Led Support helping teams understand not just what to do, but why it matters.


🌱 Enhancing Support, Not Replacing It


Video Support Plans are not a replacement for written plans, they are an enhancement.

They bring the written word to life, adding depth, emotion and clarity. They help bridge the gap between reading about someone and truly understanding them. For new team members in particular, they provide a powerful introduction, offering a direct, human connection before support even begins.


✨ A More Human Way to Support


Video Support Plans remind us of something simple but essential: the best support starts with listening. They give people the opportunity to be seen, heard and understood, not just through documentation, but through their own voice, personality and experience.
